Physical Health and Disability Service (Mid Ulster) - Northern Trust

The Physical Health and Disability Service is made up of a number of teams which provide specialist services for those with physical or sensory impairments and a Trust wide community Brain Injury Service. Who are services available to? The service is available to those who are aged between 18 and 64 years old, who live in the Northern Trust area and have a physical or sensory disability that has a substantial and long lasting effect on their ability to manage daily life. The Community Social Work Teams can assess your needs to see if you are eligible. They provide access to services at home, day care opportunities, respite for you and your carer and help to identify options for long term care. They can also provide social work support in the form of advice and counselling to help you cope with the effects of your disability and put you in touch with other professionals and organisations that could offer you support.
