Young People transitioning from Childrens Services to Adult Disability Services will be accepted to the core Adult Community Disability Teams (Physical Disability Team / Adult Learning Disability Teams) where a multi-disciplinary team consisting of Nursing / or Social Work Keyworkers, Occupational Therapists, Physiotherapists and Speech and Language Therapists will support the young person and their carer. This Multi Disciplinary team will work in partnership with Children’s Services to support a smooth transition to Adult Disability Services, until the young person reaches 18 years of age.

All those referred to the service will receive a Multi-Disciplinary assessment which may include input from Occupational Therapy, Physiotherapy, Speech & Language Therapy, Social Work and Nursing. The assessment will identify the individual’s needs and the case manager will work in partnership with the service user and family to agree how these needs can be met.
