Community Children's Nursing Service (Omagh) - Western Trust

The Community Children's Nursing Service aims to provide skilled nursing support to children with a range of health care needs in the community through : The provision of high quality, clinically effective nursing care and support to children and young people with highly complex continuing health care needs. Promotion and facilitation of care at home through the development and provision of individual packages of care for children with acute and long term health needs. Work in partnership with the child/young person/ their family/carers and statutory and voluntary services to assess, plan, implement and monitor the plan of care. Development and delivery of evidence based training plans for parents and carers to meet the clinical needs of children, in a range of settings including school, ensuring competence is safely maintained. Care coordination to support the child and family through transitions which may impact on the childs wellbeing i.e. hospital discharge, transition into school, transition into adult services. Provision of support in the home and school setting from appropriately trained children's home carers for those with most complex needs. This supports families with their child's care, supports attendance at school and facilitates short breaks/respite. Specialist nursing services for children with long term conditions i.e. respiratory conditions, diabetes, epilepsy, continence problems
