The Home Treatment Team Service provides a range of intensive mental health treatments and therapeutic services to patients aged 18-65 who are experiencing an acute disruption to their ability to function adequately in the community as a result of severe mental illness such as schizophrenia or severe depressive disorder. The treatment can take place in the patient's home, Day Hospital or Home Treatment Team House.
The crisis situation should be of such nature and severity that, without intervention, acute psychiatric hospitalisation would be the likely outcome. It also serves to prevent the need for readmission as an inpatient, or to facilitate early discharge from inpatient services.
The service adheres to the Recover Model for Mental Health, providing support and building the capacity of service users to take responsibility for their own health and wellbeing to live well despite symptoms of mental illness
Method of Referral
Referrals to the Home Treatment Team are only accepted directly through the consultant. All referrals are completed and accompanied by a risk assessment.
