Conditions Management Programme - (Armagh) Southern Trust

CMP is a work focused programme facilitated by Healthcare Professionals, such as Occupational Therapists, Physiotherapists and Mental Health Nurses. Our aim is to help you manage your health condition to enable you to progress towards, move into and stay in employment. CMP will enable you to better understand your condition(s), improve day to day functioning, increase confidence and improve prospects of returning to or staying in work. CMP is for people who are in receipt of Employment Support Allowance (ESA) or Jobseekers Allowance (JSA). To access CMP you can either speak with an Employment Advisor at your local Jobs & Benefits office, who will be happy to make the necessary referral or you can phone the office where one of our CMP team will be happy to assist. Who are the CMP team? The team have various specialist healthcare professionals with a wide range of experience in many areas of health. Depending on your needs you may work with one or more of our team members: Occupational Therapist Physiotherapist Social Worker Specialist Mental Health Nurse Exercise Professional What does CMP involve? CMP involves meeting a healthcare professional on a one-to-one basis, to discuss your specific health needs in confidence. If you both agree that CMP could be useful, then together you design a personalised action plan. The programme offers advice on: Dealing with stress, anxiety, low mood/depression Coping with pain and fatigue Relaxation techniques Communicating with confidence Developing a healthier lifestyle Exploring potential work options You can also participate in an exercise programme specifically tailored by our CMP Exercise Professional.
