What is Connect North?
Social factors such as work, money, housing problems, the challenges of managing long-term conditions or feeling lonely or isolated are just as important to our health and wellbeing as our physical needs. Connect North is a social prescribing service within the NHSCT area aiming to connect our people to the right help and support to address these social needs and improve wellbeing. Connect North has been designed and developed with our service users.

Connect North online directory of services (Population Plus) for our people
Many services exist to support people in the Northern Trust Area but often people are unaware of what is available to meet their needs. Our colleagues in Connect North have made this information publicly available through the creation of an online directory:

Connect North Link Worker Service
For people who cannot access or have no support to be able to access or use the information on the Connect North directory, referrals can be made to a Connect North Link Worker. The Connect North Link Worker can then discuss social needs and concerns with the individual and help to connect them with appropriate help and support in the local community.
