Transitions Service - The Cedar Foundation

The Let’s Get Ready School Transition programme fully involves young disabled people and their families to explore options and plan their next steps after school.

We will work together to equip you with the knowledge needed to have confidence in making decisions about your future and support you to reach your goals.

Support & Activities include :

• Practical 1-2-1 support to explore post school options.
• Completing application forms.
• Supporting with site visits.
• Exploring student finance options.
• Working with providers to ensuring the right support is in place for you.
• Engaging in personal development programmes that can include:
• Independence and Wellbeing, Resilience and Confidence Building.
• Attending Workshops - Leaving School – What Next?, Disabled Student Allowance, Positive Mental Health.

This service is available in Western, Northern, South Eastern and Belfast Trusts supporting young people aged 16 - 20 explore their options post school.

*The criteria for admission is dependent upon the contracts we hold in these trust areas.
